I (the pet owner) understand that by utilizing Nautilus Veterinary Nursing I am booking services with a certified veterinary technician and not a Veterinarian. 

I understand that by booking services through Nautilus Veterinary Nursing I am not seeking medical advice or veterinary medical treatment. 

I accept that the information and services provided to me by Nautilus Veterinary Nursing are in no way intended to diagnose my pet or prescribe medications or treatments.

Booking with Nautilus Veterinary Nursing is utilized solely to provide myself (the pet's owner) with assistance in giving the medications and treatments that have explicitly been prescribed by my pet's veterinarian or are within the scope of practice of the certified veterinary nurse. 

I acknowledge that services may not be completed if my pet's behavior becomes unsafe (ex: biting, flailing, screaming, lunging, fearfulness). I accept that the cost of service will not be refundable if this is the case due to time, cost, and loss of the appointment slot for another pet.

I, the pet owner consent to holding my pet for treatments and accept that animal behavior is unpredictable. I do not hold Nautilus Veterinary Nursing liable for any injury or accident that takes place while Nautilus Veterinary Nursing renders their services to me (the pet owner) and my pet. 

Nautilus Veterinary Nurses will not and cannot give medications or treatments without explicit documentation by a veterinarian.  Ex: asking a Nautilus Veterinary Nurse to administer 200ccs of subcutaneous fluids when the medication label provided by your veterinarian says to give only 100ccs. 

I understand and acknowledge that any information or suggestion shared with me by Nautilus Veterinary Nursing regarding my pet's care is in no way a substitute for veterinary medical treatment. Additionally, the information I am provided by any Nautilus employee is simply educational and not to be mistaken for veterinary medical advice or diagnosis. 

I agree that Nautilus Veterinary Nursing is not a substitute for the veterinary care of a licensed veterinarian and that if my pet is having an emergency, I will contact an emergency veterinary facility immediately. 

By booking with Nautilus Veterinary Nursing, I agree to all of the above and fully understand the terms and conditions.
